Standard Operational Protocol

Follow these instructions carefully to install the survey app, get survey form, complete survey form, and submit the form.

Note that you must submit the form to be considered successful. The first 400 registered prospective volunteers are guaranteed to receive N500 airtime.

Step 1: How to download and install on your phone

· An android phone is required for this process

· Navigate to google play store on your mobile phone

· Search for “ODK Collect”

· Click “install”

· Click “accept”

Ensure that you have sufficient space on your mobile, otherwise, you need to delete any app that you no longer need (To delete an unwanted app on your phone, go to "Settings", then click on "storage".

· Wait for 1-2 minutes to complete download and installation of ODK Collect.

Step 2: How to get survey form

· Confirm that your phone is connected to the internet ODK Collect app that is installed on your android mobile phone

· Open the "ODK Collect" app that was installed on your android mobile phone

· An interface appears >> click the main menu (3 dots on the right)

· Click on “General settings”

· Go to “server”

· Edit the “url” to https://odk.ona.io

· Set the username as fpca_crowd and password as crowd20 (note that the symbol between fpca and crowd is an underscore, not hyphen!)

· Go back to General settings and click "Form management"

· Under the "Periodic form updates check", click the grey text and change the option to "Every fifteen minutes"

· Next, check/confirm selection of the box for "Automatic download" and go back to main menu

· From the main menu, Click on “Get Blank Form”

· Allow for some time as the device connects to the server

· Select the relevant form named "Profile Form" OR "Pilot Food Price Survey Form", or both (since you will need both forms eventually)

· Click “Get selected”

· Allow the device some time to connect to server and get media files. When completed, a message “Download success” should pop up.

· Click OK

· Ensure that your phone GPS (Location) is turned on before proceeding to the next step.

Step 3: How to complete Profile Survey

Go to the main menu >> Click on “Fill Blank Form”

· A list of forms should appear.

· Scroll down to select “ Volunteer profile form”

· A logo page appears, swipe to the left

· Read instructions carefully

· Swipe to the left again to start inputting required information

· Always swipe to the left after answering a question until you reach the end of the survey form

· Read the instruction carefully and begin filling the form.

At the end of the form filling, uncheck (un tick) “mark form as finalized” ( Unchecking enables you to edit form in case of any mistake.

· Click “Save Form and Exit.

Note: How to edit a saved form

· On the main menu, Click “Edit saved form”

· Select the form you just filled, edit if necessary

· Check (tick) “mark form as finalized”

· Click “Save form and Exit”

Step 4: How to submit a completed survey form

· On the main menu, click on “Send Finalized Form”

· Select the form you have filled and ready for submission

· Click on “Send selected”
