Registration Form

Registration Form – was open until June 13 and is now closed 

Please note: Registration does not guarantee a spot in the training. Our team will be in touch by June 17 to let you know if you are confirmed for the training. Do not complete the Logistics and Health & Emergency forms below until your training participation has been confirmed by our team.

Visa Form

Visa Form – Due ASAP if a visa is needed

If you need a visa to enter the US, we will use the information you provide to write a letter to support your visa application.

Note: You are responsible for making sure that you have met the necessary United States visa requirements for citizens of your country. See the Applying for a Visa page for additional resources.

Mandatory FORMS 

Logistics Form – Due by July 14

To be completed after your spot in the training has been confirmed. The basic information about your education, software skills, and work experience that you provide in the registration form will be used to structure the training groups. This form will also help us determine rooming assignments and includes a media release and liability waiver.

Health & Emergency Form – Due by July 14

To be completed after your spot in the training has been confirmed. This information will be extremely important in the event of an accident or medical emergency. Please include any medical or personal information you would want an emergency care provider to know.
