All Changes of Availability & One-Week Requests must be turned in TWO weeks prior to the applicable week, starting on a Monday

All requests are at the discretion of the scheduling manager.

No schedule requests will be accepted for holidays that include: Valentine’s Day, Mother’s Day, Father’s Day, 4th of July, Thanksgiving, Christmas Eve, Christmas and New Year’s Eve.

Change of Availability: Adjust your standard availability, this will be on-going.

One Week Request: If you need to request time for a specific week, use this. Fill out *every* day on the form, not just the days you’re requesting. And always *use the Monday’s date* as the week you’re requesting. If you need Friday the 24th off, your schedule request is for the week of Monday the 20th for example.

Two Weeks Notice: We ask that you put in schedule requests two weeks ahead of time, but a “Two Weeks Notice” means you’re intending to quit. If that is the case, fill out what you’d like your final day to be. Conversely, don’t fill that out if you’re not actually quitting.
