Physicals

The New York State Education Department requires that all students entering 7th, 9th & 11th grade must have a current physical examination on file with the school.  Please be advised that NY State now requires your physician to record BMI and BMI percentages/weight status category onto the physical form.  We strongly urge you to download the school district's Required NYS School Health Examination Form page 1 and 2 and bring it to your physician for your child's next examination.  It must be dated no more than 12 months prior to the beginning of the school year in which it is required.  For example, if the first day of school is 9/6/23 then an acceptable physical would have been performed 9/6/22 or greater.  If a copy is not submitted to the nurse's office by the first day of school, then the student will be examined in school by the school physician on a prearranged date during the school year.   The Physical/Health Appraisal form can be downloaded by clicking on the form at the bottom of this page.
