New York State Health Law mandates that no medication is permitted in school without a signed Parent/Physician Administration and Authorization Form. This pertains to both prescription and over the counter medications such as Tylenol, Advil, medicated ointments, eye/nose drops, cough drops and cold remedies. If your child needs to take medication during the school day, please follow the instructions below.
1. Call the Health Office to inform us of your child's need to take medication during school hours. (631) 791-4410.
2. The completed and signed Parent and Physician Authorization for Administration of Medication form must be brought in or faxed (631-762-3710) to the office with doctor's stamp as well as signature. This form can be downloaded by clicking on the attachment at the bottom of this page. If the prescribed medication is an emergency medication such as an Epipen or Inhaler, you may provide us with a Self Medication Release Form in addition to the Parent/Physician Medication Authorization Form. Your child will then be permitted to carry or store their emergency medication.
3. A parent/guardian or responsible adult must bring the medication in to the nurse's office in the original container. This container must be labeled with the student's name and it MUST match the medication that is listed on the physician signed medication form. Please be careful of generic names - if the doctor clearly writes Advil - then the Advil brand must be brought into the office. If the doctor states Ibuprofen - then any form of Ibuprofen may be provided. The medication will be kept locked in the Health Office at all times.
If your child uses an Epipen or Inhaler, please download the appropriate action plan form to be completed by the parent/guardian physician so that we may better understand your child's individual symptoms and treatment.
