If your child needs to take any type of medication while in school (prescription or over the counter) it is required that the parent and physician complete a Parent/Physician Authorization Form. If it is an emergency medication that you and your physician feel your child is capable of self administering (such as an inhaler) then a Self Medication Release Form must be completed by parent and physican as well. Please click on the MEDICATIONS page to find these forms. Please note that these medication rules apply to any and all forms of medication including Tylenol, Advil/Motrin, Neosporin Ointment, cough drops, cold remedies, etc. Students are not permitted to carry or self administer any of these medications without the proper documentation on file in the Health Office. NYS Education Law MANDATES that all 7th, 9th and 11th graders have a physical. Physicals that are valid include any physical performed by the school physician or private physician within 1 year from the first day of the current school year. For example, if the first day of school is 9/6/23 then any physical performed as of 9/6/22 or greater fulfills the state requirement. Please provide a copy to the Health Office. This form must be submitted PRIOR to the first day of school. NYS also requests that a dental form be submitted. This form can be downloaded from the bottom of this page. Please call the nurse if you have any questions.
As required by NY State Law, all new entrants will have their vision/hearing screened by the school nurse during the school year. 7th and 11th graders who did not have a screening by their private physician will also receive a full vision/hearing screening. You will be informed if there are any abnormal findings. Please see private physical form attached below.
