 Prom Guest Approval Form Information

Are you bringing a non-CHS student as your date to prom? 

Then you need to complete and submit a CHS Prom Guest Form by FRIDAY, APRIL 11, 2025. 

Please purchase prom tickets after submitting your form.

1. Download the form: CHS Prom Guest Form (Click to download) or pick up a copy from the front office

2. Complete the form and include a copy of your guest's state issued ID. (Incomplete forms or those without a copy of ID will not be approved)

3. Turn in the form to the front office by FRIDAY, APRIL 11, 2025. NO FORMS WILL BE ACCEPTED AFTER THIS DATE!

4. Forms will be approved by school administration and you will be notified if your form is rejected or has issues. ​

5. Purchase your prom tickets in person in the front office.
