So the College has a full understanding of your daughter's medical history and needs, please complete the following medical information form. The Medical Information required is quite detailed and therefore it is broken into two sections:
Section One: Student Medical Information and Consent and
Section Two: Medical History Forms
Please fill out both sections of the form below.

MEDICAL HISTORY SECTION
Section Two of the medical information the College requires about your daughter includes information about your daughter's immunisation, specific illnesses, allergies and other medical conditions. If your daughter has Asthma or is at risk of Anaphylaxis, you will be required to upload her Asthma Action Plan and Anaphylaxis Treatment plan, so please have them ready.
Please note that PLC Sydney take Immunisation very seriously. In this section you will be also required to upload proof of her immunisation records eg a photocopy of her Immunisation Record/Booklet/or Certificate. Please have that document ready.
