Transcript Request Form

 To request a copy of your high school transcript for yourself or to be sent to an employer or a college, please complete the form linked below.

1) This form is for students that graduated from Plainville High School ONLY.  If you graduate from Plainville Adult Education please contact Kelley Whitlock 860-793-3209 or whitlockk@plainvilleschools.org.

2) A guardian cannot request this form on behalf of a child who is 18 years or older.
