Reporting Suspected Child Abuse and Neglect Forms

The form to report suspected child abuse and neglect (PS60) has been revised to include specific questions that must be answered when making a report. The revised form is available on the PGCPS website. It can also be found by clicking this link.

Directions for accessing the form are as follows:

1. Click on the Student Safety tab at the bottom of the home page on PGCPS’ website.

2. Click on the Report Suspected Child Abuse and Neglect tab and the form will automatically download as a PDF file. The reporting form is interactive so that you can complete the form on-line.

Directions for saving a completed reporting form (PS60):

1. Click the print icon,

2. Click Change,

3. See Designation, then click “Save As PDF.”

NOTE: If you do not click “Save As PDF”, then you will lose your completed reporting form.
