REGISTRATION REQUIREMENTS
In order to register a student, the biological parent or court appointed guardian MUST upload the following documentation at the time of registration:

- Birth Certificate
- Student’s current immunization records
- PROOF OF RESIDENCE (1 item needed)
MUST provide ORIGINAL document.

- Settlement papers
- Lease/Rental Agreement (Private party lease must be notarized)
- Deed of Property
- Mortgage Statement (within last 30 days)
- Current Property Tax Bill
- ADDITIONAL DOCUMENTATION (2 items needed)
MUST provide ORIGINAL document received within the last 30 days via US Postal mail which includes parent/guardian name and current address. Online documents and/or printouts will not be accepted.

- Paystub
- Non-cell phone bill
- Utility bill
- Credit card statement
- Bank statement
- Mail from government agencies (excluding PGCPS)
- Hospital bill
- W-2 form
- Auto, home or health insurance policy
- Student’s last report card or withdrawal packet
- Parent/Guardian picture ID
- If applicable, current IEP (Individual Educational Plan) or 504 plan
