AS A CONDITION TO PARTICIPATING IN THE 45TH GIRLS JUNIOR PGA CHAMPIONSHIP (THE “CHAMPIONSHIP”), WHICH IS SCHEDULED TO BE HELD AT VALHALLA GOLF CLUB IN LOUISVILLE KENTUCKY (THE “VENUE”), FROM JULY 25-30, 2021, YOU (“PLAYER”) AGREE TO THE FOLLOWING:

GIRLS JUNIOR PGA CHAMPIONSHIP: TERMS AND CONDITIONS OF ENTRY


By registering for the Championship, on behalf of the “Participant”, the Participant and the Participant’s Parent and/or Legal Guardian acknowledge and agree that they have READ AND ACCEPT THE TERMS AND CONDITIONS OF THIS CONDITIONS OF ENTRY AGREEMENT FOR THE 2021 GIRLS JUNIOR PGA CHAMPIONSHIP IN ITS ENTIRETY:

This is the formal Declaration of Intent of Participant to participate in The Girls Junior PGA Championship, July 27-30, 2021 at Valhalla Golf Club in Louisville, Kentucky.

Age limit: Open to girls who have not reached 19 years of age on or prior to July 30, 2021. Application for entry is subject to acceptance by the The Professional Golfers' Association of America (“The PGA of America”).

Applicant agrees that, in the event of its acceptance, it may be rejected at any time thereafter. The Committee reserves the right to adjust dates, format and other details to fit conditions prevailing at the time of the Championship. The applicant and the parent/guardian certify that:

the applicant is an amateur player, as defined by the USGA;

all regulations, rules and conditions of entry have been read and understood and approval has been granted in submitting this entry; and

the information given in this application is true to the best of their knowledge.

It is a condition of participating in this event that the competitor is not permitted to use a motorized golf car during the event competition. Notwithstanding the foregoing, The PGA of America is committed to conduct golf competitions within the parameters of the Americans With Disabilities Act. Players who request relief under the Americans With Disabilities Act and seek a reasonable accommodation for their disability by the use of a motorized golf car during the event competition shall be required to submit their request in writing on or before the entry deadline date for review and determination. A parent or legal guardian is encouraged to accompany their children to the Championship to provide adult supervision. Players are not supervised 24-hours a day. Players and parent/guardians both acknowledge that none of the parties listed in the following parentheses (The PGA of America, Valhalla Golf Club and any co-hosts and all of each company's respective officers, directors, employees, agents, subsidiaries and affiliates) are providing adult supervision to the players at any time other than while the players are on the golf course. In consideration of the applicant's participation in The Girls Junior PGA Championship:

The applicant and the parent/guardian hereby acknowledges that they each assume all risk and danger incidental to participation in the event (both to the applicant/parent/guardian and to third parties as a consequence of the applicant’s actions) including, but not limited to, the risk of exposure to or contracting a communicable disease or illness, and release the Kentucky PGA Section, The PGA of America, PGA Tournament Corporation, Inc., Valhalla Golf Club, Sponsors, any co-hosts and all of each company's respective officers, directors, employees, agents, subsidiaries and affiliates from any and all liability from such cause and from accidents, injuries, or illnesses sustained by the applicant or parent/guardian in connection with the Championship or travel to the Championship; and

Unconditionally and irrevocably grants and assigns to The PGA of America, without limitation, the right to use their name, image, voice and likeness in television, radio, motion picture, photographic, internet, social media, electronic and all other media, whether now in existence or hereafter created, in connection with their participation in the Championship and/or other events conducted in conjunction/connection with the Championship. For the avoidance of doubt, this grant of rights shall authorize The PGA of America, any of its affiliates, subsidiaries, and assigns, to make any and all use of the player’s name, image, voice and likeness in connection with: (1) advertising, promoting or publicizing the Championship, any television or electronic broadcasts or radio broadcasts of the Championship, any subsequent Championships, The PGA of America, or any events sponsored or cosponsored by The PGA of America and the game of golf in general and/or the players; (2) live or recorded telecasts, television programs, radio broadcasts, internet broadcasts, or other electronic transmissions or programming during golf tournaments; (3) broadcasts or exhibitions on a recorded basis as part of a program, motion picture, electronic transmission programming or video recording of the Championship, or year-end summaries of sports award programs or any news or sports news programs; (4) live or taped non-exclusive updates in any form of media. The applicant and the parent/guardian acknowledges and agrees it is their intent to provide the rights outlined herein and to intentionally forego any rights to review and/or approve any recording or transmission. In the event, a player or any person or entity with a player, record and/or otherwise capture, via any device and/or manually, whether at the site of the Championship or remotely, any data, content, images, video or statistics, in any form, related to and/or resulting from a player’s participation in the Championship (“player data”) the player hereby agrees to not use such player data for any commercial purpose, as such is defined by The PGA of America, without first obtaining the approval of The PGA of America, such approval at The PGA of America’s sole discretion.

As a condition of entry all players expressly release and discharge The PGA of America. its affiliates, officers, directors, employees, members, sponsors and the host site from and against any claims for injuries or illnesses sustained incidental to participating in the event. Players agree that they choose to participate in the event at their own risk and players assume the risk of injury or illness in association with participating in the event.


PLAYER CODE OF CONDUCT

  • Players will not show unsportsmanlike conduct, including cheating, club throwing, abuse of golf course property, physical abuse, threat of physical abuse, or verbal abuse towards other players, parents, coaches, sponsors, host golf course staff or members, PGA staff and officials, and volunteers.

  • Players will adhere to the PGA dress code at the host facility

    • PGA Dress Code includes:

      • Hats/visors must be worn with brim forward

  • Players will not use or associate with drugs, alcohol, vaping, or smoking or chewing tobacco products

  • Players agree to care for the golf course and leave it in better condition than they found it

  • The Championship Committee may assess varying penalties at their discretion pending the severity of the code of conduct breach.

SPECTATOR POLICY

  • Spectators agree to remain on the cart path at all times.

  • Spectators agree to refrain from having any contact with players during their Championship Rounds.

  • Spectators agree to defer the review and enforcement of rules decisions to the PGA of America Rules Committee Members.

  • Spectators agree to demonstrate sportsmanship, respect, and a positive attitude.

  • Spectators agree to abstain from caddying during Championship Rounds.

  • Spectators agree to abide by any rules and regulations of the host facility (dress code, cell phone policy, etc.)

  • Failure to comply with the spectator code of conduct can result in the immediate removal of the individuals involved for the remainder of the Championship. Warnings may not be issued pending the severity of conduct breach.

LIMITED MOBILITY SPECTATOR POLICY

  • Limited mobility spectators will not be able to rent a cart under any circumstance. The PGA of America will store a rented scooter for you on-site if needed.
