PHYSICAL EXAMS
Physical exams are required at entry into the school district and in grades K, 6th, and 11th. Your child's primary care provider should complete the physical exam. If you do not have insurance to cover the cost of the exam, please contact the school nurse for assistance.
DENTAL EXAMS
Dental exams are required at entry into the school district and in grades K, 3rd, and 7th. Your child's dentist should complete the dental exam. If you do not have dental insurance to cover the cost of the exam, please contact the school nurse for assistance.
