All students in Kindergarten, 6th, and 11th grades are required to have completed physical exams.
All students in Kindergarten, 3rd, and 7th grades are required to have a completed dental exam.
If your student requires medication to be administered at school (This includes emergency medications such as inhalers and Epi-Pens) please submit the necessary paperwork and drop off the medication to the Nurse as soon as possible. See below for our Medication Policy.

MEDICATION POLICY
ALL medications, including prescription and over-the-counter medications, herbal supplements, and essential oils, require:
1) A signed order from a licensed prescriber and a parent signature.
2) Medication must be in the original container.
3) Medication must be brought in by a parent or guardian, and a signature is required at the time of drop-off.
Epinephrine auto-injectors, inhalers, and eye drops may be brought in with the student, with a signed order from the physician.
It is the parent's responsibility to notify the school immediately and submit a new form if there are changes in medication, dosage, time of administration, and/or the prescribing physician. Parents may request two labeled containers from the pharmacist, one for school and one for home. Medication can only be administered at school during the regular school day.
