Waiver of Informed Consent: You are being asked to participate in a research study about maximizing outreach to individuals in rural and remote areas and distance consultation practices in Orientation and Mobility (O&M) or in teaching individuals with visual impairments. The Principal Investigators include Dr. Amy Parker, Ms. Mary Tellefson, Ms. Hong “Nana” Phangia Dewald, Dr. Holly Lawson, and Ms. Katherine Ericson from the Department of Graduate School of Education, at Portland State University (PSU) in Portland, Oregon.

This research involves reviewing or listening to a presentation about an online learning module about distance consultation approaches and then choosing to take an online or paper based survey. You are making a decision whether to participate in this study. The survey should take no more than 15-20 minutes of your time. It is anonymous. You may stop taking this survey at any time that you wish with no penalty. At the end of the survey, you will also be invited to participate in a focus group with other vision professionals. You are welcome to participate in only the survey without joining the focus group.
