Student Enrollment Forms

Enrollment forms must be filled out by the student and their legal guardian. 

The enrollment form serves as parent permission for (1) students to participate in MESA and (2) for Oregon MESA to contact their student with our programs and opportunities.  We also use student demographic information (names redacted) to apply for funding that allows us to continue to bring programs to students.

 If you do not have the ability to fill out an online form at home, please see the printable form options (below the online forms).  It is your responsibility as the student to get your completed forms to your advisor, and to have your parents read and sign the liability waiver at the end.
