Teacher-Librarian Collaboration Form

Below is the collaboration form that will serve as an aid in our collaborative teaching efforts. Please fill out the form as completely as possible and attach any pertinent materials that will be used in our lessons. You can email the information to me at mbeyel@pbcschools.org, drop it by the library, or leave it in my mailbox in the front office. I will review the materials and contact you ASAP to set up a collaborative planning meeting with you.
