Which mandated health forms need to be completed?
The answer will depend on the student’s grade. Every student needs to turn in a health screening form. So at minimum, you know a health screening is due every year.
In addition to the health screening exam, students in Kindergarten, 3rd grade, and 7th grade need to turn in a dental form. Students in Kindergarten, 6th grade, and 11th grade need to turn in a physical form.

How early can I have the form completed?
Health Screenings must be completed during the school year. So to count for the current year, they must be dated after July 1st. For example, a health screening must be dated after July 1, 2025 to count for the 25/26 school year.
Dentals and physicals can be accepted from the year prior. This means that a dental or physical can be dated after July 1, 2024 and still count for the 25/26 school year.
How can I submit forms?
We have a number of different methods for you to send forms to us. You can email, fax, upload, or mail the forms. A member of Pupil Health will confirm for you once the form is received. If you don’t hear back from us, we haven’t received it yet.

What is the difference between a physical and a health screening?
Health screenings track the condition of a student’s vision, hearing, height, weight, and Body Mass Index. They also check for scoliosis of curvature of the spine in middle school students. One way to think of it is that health screenings check to see that your student can see and hear the lessons being taught. Health screening can be completed at a free PA Virtual site, or done by a doctor during a well check.
Physicals are additional exams that are more extensive than screenings. They are system checks (such as heart, lungs, and more) that must be completed by a physician. They review a student’s overall health and are mandated for students in Kindergarten, 6th and 11th grade. These exams can only be done by your doctor (PA, DO, CRNP, MD) and cannot be completed at one of PA Virtual’s free Health Screening sites.
I don’t have my form filled out, but I have an appointment date. What should I do?
Let us know when you have an appointment date. You can email your school nurse the date, or you can upload the appointment date just like you would the form. Once the appointment is done, please send us the completed form.

When do I need to have my forms in by?
The Pupil Health deadline to turn in mandated health forms is July 1, 2026.
If your student’s exam is scheduled after July 1, 2026, it is essential that you contact us prior to July 1, 2026 to let us know the new appointment date.
