STEP 3: SUBMISSION OF POST-TRAINING REQUIREMENTS

Upon completion of your Internship/Practicum, please complete the following requirements:

a.) Accomplish the Exit Internship Evaluation.

b.) Upload your Certificate of Completion and Job Accomplishment Report (JAR) using the designated submission link.

c.) After completing all required steps, submit the original copies of your Certificate of Completion and Job Accomplishment Report (JAR) to the Registrar's Office.

Please allow up to five (5) working days for the validation and verification of your uploaded documents. Once the review has been completed, you may check the status of your submissions through the OJT Document Tracker.

Kindly ensure that all requirements have been completed and validated before proceeding with the submission of your original documents.
