STEP 2: UPLOAD YOUR PRE-TRAINING REQUIREMENTS

 Pre-Training Requirements:

a.  Acceptance Form (downloadable)

b.  Notarized Memorandum of Agreement (downloadable)

(For companies with existing Memorandum of Agreement (MOA), kindly upload the OJT Acceptance Form in the designated MOA field unless otherwise the company has special instructions.)

c.  Copy of the Endorsement Letter received by the Company Representative

The file should be in PDF form with a maximum file size of 5MB each with a filename format "Document Type - Surname, GivenName". (eg. MOA - Doe, Jane.pdf)

Documents will be uploaded during official working days and hours only (Monday through Friday from 08:00 AM to 05:00 PM). Documents sent during weekends, holidays, or after working hours will be processed the next working day. Validation and checking of uploaded documents will take up to 5 working days.
