ON THE-JOB-TRAINING

On-the-job training is crucial in preparing students for the workforce. With OJT, students not only get the opportunity to acquire practical knowledge, skills, and work values in reputable establishments or industries in our country. It is an avenue for applying, enriching, and enhancing the acquired knowledge in the school in their particular field of endeavor. In some cases, OJT leads to employment opportunities, allowing a graduate to hit the ground running early.

STEP 1: 

APPLICATION FOR OJT WAIVER AND ENDORSEMENT LETTER

The application schedule is set for every Monday, Wednesday, and Friday, running from 08:30 AM to 11:30 AM, excluding holidays.


List of Requirements:


a.   Most recent copy of Job180 Resume

b.   Completely filled out OJT Waiver in triplicate copy. (downloadable) 

c.   Original NBI Clearance

d.   Scanned Copy of Parent/Guardian’s ID with signature

e.   Notarial fee (Php200.00) 

NOTE: Requirements should be in a clear plastic L Folder. 


Claim the Endorsement Letter for the Chairperson’s Signature. It must be claimed within 10 days after filing. The schedule is every Monday, Wednesday, and Friday from 03:00 PM to 04:00 PM. Bring your school ID or any valid ID for verification. Visit the OJT Document Tracker for the status of your OJT Documents.

After the student’s application is accepted, he/she must upload the Pre-Training Requirements duly signed by the company representative before the student begins his/her training. For companies that already have a Memorandum of Agreement (MOA), kindly upload the OJT Acceptance Form in the designated MOA field. Please see below the list of requirements. The file should be in PDF form with a maximum file size of 5MB each with a filename format "Document Type - Surname, GivenName". (eg. MOA - Doe, Jane.pdf)


Uploading of documents will be during official working days and hours only (Monday - Friday from 08:00 AM to 05:00 PM). Documents sent during weekends or holidays or after working hours will be processed the next working day. Validation and checking of uploaded documents will take up to 10 working days.  Pre-Training Requirements:

a.  Acceptance Form (downloadable)

b.  Memorandum of Agreement (downloadable)

c.  Copy of the Endorsement Letter received by the Company Representative


Upon completion of the Internship/Practicum, answer the Exit Internship Evaluation.  Upload the Certificate of Completion and Job Accomplishment Report (JAR) through the link below.  Submit the ORIGINAL COPY of your OJT Certificate of Completion and Job Accomplishment Report to the Registrar's Office. Validation and checking of uploaded documents will take up to 10 working days. Visit the OJT Document Tracker for the status of your OJT Documents.


Post-Training Requirements:

a.  Certificate of Completion/OJT Certificate/Training Certificate

b.  Job Accomplishment Report (JAR)

c.  Exit Internship Evaluation
