Secondary transition refers to the process of a student getting ready to exit from student life and move into adult life.  The process occurs over a period of several years and focuses on increasing independence skills to prepare students to transition into adult life and post-school activities.  Starting at age 12, transition planning formally begins and includes the student, family, school staff, and supporting agencies.  Transition planning helps determine the types of transition services needed to prepare students for post-school activities such as:

• College or university programs

• Career and technical (vocational training)

• Employment

• Continuing adult education

• Independent living

• Community participation
