Secondary transition refers to the process of a student getting ready to exit from student life and move into adult life. The process occurs over a period of several years and focuses on increasing independence skills to prepare students to transition into adult life and post-school activities. Starting at age 12, transition planning formally begins and includes the student, family, school staff, and supporting agencies.
