The Pennsylvania Public School Code Section 1418. Medical Examinations of Teachers and Other Persons requires that “teachers, janitors, cooks and other cafeteria help and all others employed at schools have a pre-employment medical examination. The law does not allow modification of this section.

PA Leadership Charter School only accepts this form, and it must be completed by a physician. An approved physical form is required to complete the employment process and the form must be presented in order to meet PDE requirements. Please note that a TB test already administered by a physician may be provided as an addendum to the approved physical form above as long as the TB test was administered and read within 3 months from the employee start date.
TIP ON SCHEDULING PHYSICAL – If seeking a physical from your own physician, let them know it is for employment purposes and they will tend to schedule it within just a few days of the request. If you do not have a prescribed physician, you may obtain this physical at a business such as CVS (or any other pharmacy that has “mini” clinics) or urgent care facilities.

Instead please upload the file to our secure upload link (ShareFile). All file transfers through the ShareFile service are encrypted using 256-bit SSL (Secure Sockets Layer). This is the same security used by banks and many e-commerce sites.
