Patient Eligibility

Who Can Refer a Patient?

What happens next?

We know this may be a quite difficult time, so we encourage you to contact us at anytime with any questions.

Routine Home Care

Available wherever the patient considers home, including a residence, skilled nursing facility or assisted living community.

Continuous Home Care

In case that the patient's health dramatically declines and needs close medical attention, Our Caring Hands staff can provide continuous care in the home. When the crisis is over, the patient returns to routine home care.

General Inpatient Care

When the medical symptoms or pain cannot be controlled at home, Our Caring Hands can accommodate the patient at a hospital or other inpatient facilities to stabilize the patient before returning them home to routine home care.

Palliative Care

Also referred to as ‘comfort care.' This treatment enhances comfort and improves the quality of an individual’s life who is facing a serious illness but may not qualify or be ready for hospice care. The primary goal is focusing on treating illness, addressing symptoms and improving the quality of life. Our palliative care aims to support the patient and their family as needed, including finding community assistance

Bereavement Support

Bereavement support is given for 13 months after the patient passes for all of our families and caretakers.
