Work Permit Instructions
Once you have been offered a job, you will need to fill out the work permit application form. Please clink the link above.
1. Print the form and fill out the section titled "Minor's Information"
2. Have your parent or guardian sign where it says "To be filled in and signed by a parent or legal guardian"
*Do these two steps before you go to the interview and take the form with you.*
3. Fill out the school information as follows

4. Have your employer fill out the section "To be filled in and signed by employer." YOUR PERMIT CANNOT BE FILED WITHOUT THIS INFORMATION.
After your application form is completed please drop it off with Ms. Clark in the Library and she will process your application and submit it to administration for approval. Approved permits will be ready to be picked up 24 hours after you drop it off.
