This form is to be used for requesting permission to enroll in an
Out of District class through an accredited school or program
(including online courses) for remediation of D or F  grades. 

This form must be completed, submitted with counselor approval prior to the start of a non-OPUSD remediation course. After you submit the form, your counselor will review the form and notify a decision via email.
