Use this form to inform the division that you plan to take a leave of absence for one semester or longer. Note: this form is for divisional planning purposes only, and is not a request for approval. You will need to reach out to HR to officially request the leave.

To initiate requests, fill out the NYUAD AdminLink Service Request Form (consider bookmarking it for easy access). To ensure that the process runs smoothly, please consider all requests to be made in advance of your expected or envisioned needs. Lead times for each task can be found in the NYUAD AdminLink User Manual and on the request form.
