PROJECT NARRATIVE
Deaths from cancers arising from the gastro-esophageal junction represent a major public health burden, yet the factors that drive early changes in this region remain poorly understood. We aim to gain a clearer understanding of the effects of secondary bile acids produced by bacteria in the gut on stem cells in gastro-esophageal region. This knowledge can in turn be translated to future efforts to target the gut microbiome to prevent gastro- esophageal cancer.

PROJECT NARRATIVE
To address fundamental questions regarding the cell-of-origin of distinct classes of gastric cancer, we will integrate novel mouse models, organotypic cultures of human and mouse cells using state- of-the-art single cell profiling and spatial transcriptional analyses. We will also explore the related issue of disease development by defining key transcriptional mediators of the progression to gastric cancer and then perform experimental validation. Taken together, our studies will address and resolve longstanding debates over the origins of gastric cancer while also defining possible targets to prevent cancer initiation.

Gastric cancer is common globally. We seek to develop and characterize model systems (iPSCs, organoids) to reveal how gastric intestinal metaplasia and dysplasia, precursors to gastric cancer, form. In so doing, we will unravel new insights into potential applications for prevention, early detection, screening and surveillance.

Although stem/progenitor cells at the gastroesophageal junction have been shown in preclinical models to contribute to Barrett’s esophagus, their precise characteristics and mechanisms involved in neoplastic progression remain undefined. This proposed study will not only provide the molecular profile of mouse and human Barrett’s stem/progenitor cells but also offer insights into the role of p53 mutation and tuft cells in Barrett’s metaplasia and progression. The knowledge gained will provide a framework for identifying new therapy for reducing the risk of Barrett’s progression towards malignancy.

The molecular features of esophageal adenocarcinoma and intestinal gastric cancer suggest they are a single disease from a common and unusual cell-of-origin. Therapeutics that target this common cell-of-origin could contribute to strategies for the elimination of these lethal cancers.

PROJECT NARRATIVE Gastric cancer develops within a carcinogenic cascade from pre-cancerous metaplasia to dysplasia and adenocarcinoma. While the pre-cancerous metaplasia is possibly reversible, a key cell population which is responsible for mucosal recovery or neoplastic progression is to be defined. In this study, we will define mechanisms that control the cell lineage conversion of reparative SPEM cells, which are a major cell component of dysplasia, towards incomplete IM and more cancerous cell lineages.
