Hard Copy LDO Acknowledgement Form

Some schools prefer their employees utilize the hard copy acknowledgement form. School admin teams will be responsible for tracking and obtaining all hard copy forms and certificates for each employee/volunteer every year. NOTE: NW Complex Areas are not permitted to use the hard copy acknowledgement form.
