During this time of Distance Learning ALL services will be provided through telecommunications such as phone, Google Meets, or Google Hangout. In order to participate in individualized services a Consent for Social Worker Services Form will need to be completed and returned. (click on below doc) If for any reason you are unable to complete this form and return, there is an option for verbal consent and a copy will be emailed to the guardian.
