Health Forms and Requirements

Medical Exam Form

*Required for Kindergarten, 6th, 9th grades and all out-of-state transfer students

Forms are due no later October 15th (or 30 days after enrollment if transferring). Any student without the required documentation by the due date will be excluded from attending school until the proper documentation is received.

Vision Exam Form

*Required for all Kindergarten and out-of-state transfer students

Forms are due by October 15th (or 30 days after enrollment if transferring from another state)

Dental Exam Form

*Required for Kindergarten, 2nd, 6th and 9th grades

Forms are due by May 15th.

Certificado Médico en Español

*Requerido para estudiantes que ingresan a Kindergarten, 6.° grado, 9. ° grado y estudiantes de transferencia.

Los documentos son debidos por el 15 de octubre (o 30 días despues de inscripción para estudiantes de transferencia) o los estudiantes serán excluidos de la escuela.

Sports Physical Form

*Required for any student wishing to participate in school sponsored sports

Medication Authorization Form

*Required for ANY medication to be given at school, including over-the-counter

Allergy Action Plan

*For any student with severe allergies to food, bee stings, etc.

Pre-K student health requirements (first year of enrollement):

Medical examination form completed by a licensed health care provider
Proof of mandated vaccinations required by IDPH for entrance to Pre-K

Kindergarten student health requirements:

Medical examination form completed by a licensed health care provider (due by October 15th)
Proof of mandated vaccinations required by IDPH for entrance into Kindergarten
Vision exam form completed by a licensed eye doctor (due by October 15th)
Dental exam from completed by a licensed dentist (due by May 15th)

2nd Grade student health requirements:

Dental exam form completed by a licensed dentist (due by May 15th)

6th Grade student health requirements:

Medical examination form completed by a licensed health care provider (due by October 15th)
Proof of mandated vaccinations required by IDPH for entrance into 6th Grade
Dental exam form completed by a licensed dentist (due by May 15th)

9th Grade student health requirements:

Medical examination form completed by a licensed health care provider (due by October 15th)
Proof of mandated vaccinations required by IDPH for entrance into 9th grade
Dental exam form completed by a licensed dentist (due by May 15th)

All Out-of-State Transfer students entering an Illinois school for the first time:

Medical examination form completed by a licensed health care provider (must be IL form) (due within 30 days of enrollment, or October 15th of the current school year, whichever is later)
Proof of mandated vaccinations required by IDPH for the student's current age/grade
Vision exam form completed by a licensed eye doctor (due within 30 days of enrollment, or October 15th of the current school year, whichever is later)

*Sports physical forms are required to be filed for any student who is participating in a school sponsored sport. This needs to be turned in prior to try-outs and/or practice. The medical examination form is acceptable for 6th grade, 9th grade, and transfer students as long as the doctor has checked "yes" to participation in sports.
