To provide optimal care to you child, please keep us up to date on any chronic conditions, injuries or needs that your child may have. 

                                                  Physical Education:

The Health Office requires documentation from a physician to excuse a student from Physical Education due to illness or injury. 

Please provide our office with adequate information, documentation and date of follow up appointment.

A doctor's order is required for return to activities.

Students will be excused from both Physical Education and Recess due to our district policy.
