KINDERGARTEN IMMUNIZATION REQUIREMENTS

DTap 4-5 doses

IPV 3-4 doses

MMR 2 doses

Hepatitis B  3 doses

Varicella  2 doses

Students who are not fully immunized will not be able to attend school after September 21, 2023.  To avoid interruption in your child's attendance, please consult with your child's health care provider to ensure they are to date with all immunizations.

If your child is uninsured, you may call the  Onondaga County Vaccine Clinic at 315-435-2000 or visit the link below to see if you child is able to receive their vaccines at the clinic.

Incoming Kindergarteners must submit the following information:

• Health History and Food Allergy Form

• An updated physical exam

• Documentation of all immunizations

• Dental Certificate (requested)

• Additional forms as necessary include: Allergy Action Form, Seizure Action Plan, medication order
