If you are looking to change your child's schedule here at FRC, please fill this form out and either email it to myself (Kristina) or hand it in to the FRC staff.

Please take note that this form requires 2 week notice before a change can occur.

This form is to be filled out if you no longer wish to send your child to the FRC program.

Health Assessment Form

This is the health form we need for each child attending FRC. We ask that we get any updated ones after a physical has taken place. Part 1 is for you the parent/guardian to fill out and Part 2 is for the doctors office. (If your child requires medication while at FRC ex: inhaler, we require a treatment plan for said medication signed by the doctor as well as the guardians.)
