Step 1: Complete a Work Permit Application
Contact the front office at Monte Vista for your California Work Permit Application (CDE Form B1-1).
Step 2: Take the Work Permit Application to your employer
Have your employer complete and sign the section titled, "To be filled in and signed by employer".
Step 3: Bring Completed Application to Your School
Once your application is completed, bring it to the front office
Step 3: Receive your Official Work Permit
Monte Vista will print your official work permit. You can sign the student field of the executed work permit and deliver the fully completed work permit to your employer.

Contact the California Department of Industrial Relations to review steps on completing the application.
Once application is completed, you will need to submit to your school site (rcartmill@nmusd.com) for review and processing of the school portion.
The school site will complete the Attendance, Grades, and Health fields, and return the application to the parent. It is then the parent's responsibility to contact the appropriate agency authorized to issue Entertainment Work Permits.
