The Administration of Medications, Medical Interventions and Chronic Care policy can be accessed here. If your child requires emergency medication to be administered as needed or will require medication during the school day it will be necessary to complete the following forms:
● 301A - Administration of Medication (to be updated annually and/or if there are changes to the student’s medication)
Section 1 - Administration of Medications - to be completed by parent/guardian
Section 2 - Administration of Medications - to be completed by physician
If your child has an allergy that places them at risk for anaphylaxis the following documents must be completed in addition to form 301A.
Anaphylaxis Management Plan - please review the Guidelines for Anaphylaxis Management in Schools and complete pages 13-17 of this document and return them to the school
Anaphylaxis Emergency Plan - please complete and return with a recent photo
