Request for Addition of Recruitment Site To CPMS [NIHR only]

This form should be submitted when recruitment needs to be uploaded at a site that does not exist in CPMS.

Request to add new Organisation to CPMS [NIHR only]

This form should be submitted when a new funder or sponsor organisation needs to be added to CPMS.

This form should be submitted when a study record in CPMS needs to be deleted due to data quality issues
