How to Register & Request Accommodations
Admitted students to the College who have a disability may register with the Accessibility Services Office by following the steps listed below. Once eligibility is established and the student completes the registration process, they may request accommodations to ensure equal access to College programs and activities.
Registration Process
Submit an Application for Services.
Students may apply for services by completing the online Application for Accommodations form
Student must read through our Accessibility Services handbook to thoroughly understand our processes.
Students may request a paper copy of the application by contacting the Accessibility Services Coordinator
Schedule Intake Appointment with the Accessibility Services Coordinator.
Upon submitting the Application for Accommodations, an Accessibility Services staff person will reach out to the student to schedule an intake appointment which can be done in person, by phone or Zoom meeting.
Submit Documentation of Disability.
Students can print a copy of our Provider Form for Documentation or pick up a copy of the form in our offices and have their provider complete it and return. If the student has an IEP or 504 Plan, this can be uploaded to our Application for Accommodations form online, emailed or brought to our offices.
The Accessibility Coordinator will review the documentation and understand how the disability may impact a student and make informed decisions on accommodations.
Determination of Eligibility
Upon review of the findings from student intake appointment, and the documentation of disability, a determination of eligibility will be made. Students eligible for services will be given written notice of approved accommodations. In cases where a student is not eligible for services, appropriate campus or community referrals will be made.
When the registration process is complete, the student will be emailed their accommodation letter to forward to each of their instructors.
Students must complete the registration process with the Accessibility Services Office prior to receiving services.
The student is responsible to renew their accommodations each semester they are taking classes.
Releasing information
If a student only wants to authorize the Accessibility Services Office to communicate with a particular point of contact (for example you want Accessibility Services to speak with your mental health counselor or send specific information to another point of contact), you can complete the Accessibility Services Authorization to Release/Obtain Information form.
If a student wishes to obtain a copy of their disability documentation that was submitted to NICC, there is no need for a release as long as the request is coming directly from the student. Students may need proof of prior accommodations and/or their documentation for advanced testing in their major or to transfer to another college. Our staff can assist in sending this information directly to the student or to their next college of choice Accessibility Services office.
