MARCHING BAND MEMBERSHIP FORM

Please get this in as soon as possible, but no later than June 30th.  Membership to the marching band may not change (additions or deletions) past June 30th.  

MARCHING BAND FEES (Updated 2024 fees form 4/29)

This form is due ON THE FIRST DAY OF BAND CAMP.  Included on the form are various mandatory and optional fees for participation during the season.  This form should be returned to Mr. Reynolds on the first day of camp – do not mail the form, and please turn it in person as requested on the first day of camp.

PHYSICAL EXAMINATION FORM

This form is due to the school nurse by July 31st.  Any student participating in an extracurricular activity must complete this form as per district policy.  This form should be submitted to the nurse along with the parent permission form. (I will send a reminder email about this if any information is needed)

ASPEN REGISTRATION INFORMATION

Parents of band members must sign up for an account and fill out all required forms in Aspen: Parent Permission Form, Consent to Random Drug Testing, Steroid Testing, and the form regarding concussions.  Please note: ASPEN Registration is generally opens during the first week in July through July 31st for fall sports. (I will send an email out to everyone once it is officially open for the fall season)

BAND CAMP CHECKLIST

This file includes a checklist of required and recommended items that you will need for our full-day rehearsals starting on August 5th, 2024.
