The counseling department will be glad to assist you with transitioning your student to our school.

In addition, we will need you to provide our school with the following:
1. Original birth certificate - Please bring in the original birth certificate and we will make
a copy for our records.
2. Proof of residency in Sandy Hook/Newtown - Two forms: Homeowners must present one of the following: a current mortgage statement/bill, a copy of the Newtown
home property tax statement or the title page of the current homeowner’s insurance policy.
OR
Renters must present a current rental/lease agreement or notarized letter from landlord or owner acknowledging the parent/guardian and student’s residence
AND
One current utility bill (electric, gas, or phone)
3. Medical Information - A current physical and with immunization record is required per CT guidelines for attending school.
