Step 1: Staff member identifies a student struggling academically, behaviorally, or socially, and collects evidence of the issue(s).
Step 2: Staff member contacts parent/guardian to notify them of issues.
Step 3: Staff member completes the Google "Intake" Form
Step 4: The student is scheduled for discussion at the next I&RS Meeting.
Parent is invited to attend this meeting.
Teacher provides more in-depth information.
Data is analyzed.
Action Plan is created with targeted interventions and SMART Goals.
Step 5: Interventions are monitored for 6-8 Weeks.
Staff Member(s) takes data on how interventions are progressing.
Step 6: After 6-8 Weeks, I&RS team meets on the student again.
At this meeting one of the following determinations will be made;
Case is closed.
Action Plan Continues.
New Action Plan is developed.
Student is referred to CST.
