Required Forms

All incoming students must complete and submit the required forms below. To access the forms, you will need to be logged into your NEC Google profile.

Students whose forms are not received by the first day of spring classes may experience a delay in registration.

Academic Record Release Form

As part of the FERPA (Family Educational Rights and Privacy Act of 1974, as Amended) policy, information regarding your educational records, including grades, may not be released without your expressed written consent.  However, NEC acknowledges that parents can and should play an important role in the student’s continuing life in education.

By granting permission to release information regarding your educational records (excluding transcripts and disciplinary/conduct records), you authorize copies of final grades and correspondence regarding your academic record, bills and payment arrangement, and/or Financial Aid documents and loan information to be released to or discussed with both you and your parent(s)/guardian(s) by appropriate officers of New England Conservatory.

This request may be updated or revoked, in writing, at any time.

Check-in Form

Incoming students are required to provide the College Registrar's Office the following information prior to the beginning of the school year:

·        Permanent/Family Address (this can NOT be your address on campus at NEC)

·        The address where you will reside while attending school this Fall (this is where you live in Boston during the school year, whether on campus or off)

·        A US mobile phone number

·        Emergency Contacts (this is who we will contact if you are in an emergency -- it can NOT be your own contact information)

The Office of Academic and Student Affairs has developed an online tool to collect this information so you will not have to complete this process in person. A link to that tool can be found HERE.

Please note that this process is a requirement of both the state of Massachusetts and the City of Boston. The deadline for submitting this form is August 15, 2024.

Massachusetts Motor Vehicle Acknowledgement

Please fill out the Massachusetts Motor Vehicle Acknowledgement Form. This form is required for all students even if you do not plan on bringing a car to Boston. Follow the link below to the form. You will need to log on using your NEC credentials (same as your NEC email username and password).

Recording and Photography Release

Please fill out the NEC Recording and Photography Release Form. Follow the link below to the form. You will need to log on using your NEC credentials (same as your NEC email username and password).
