Undergraduate Students

For Undergraduate Students: an eForm is available in Campus Connection. Please let the Curricular Human (currently Kyle) know, or just ask Kyle to do it.

Graduate Students

The Change to Advisor/Supervisory Committee form should be submitted (see Graduate School Forms). If the advisor has left NDSU or retired, that change can be made over email to Melissa Selders-Ortez.
