Travel Reimbursement Information
Travel Information for Pittsburgh Tech Crawl June 17th & 18th:
If you are over 50 miles away from the hotel, the PA Smart Grant will cover hotel, mileage, tolls, travel-day meals that aren’t apart of the program. It may even cover flight instead of mileage depending on where in the state you are coming from. please do the travel pre-approval.
Western PA Residence: Since the program may go until 8:00 pm on Monday night, if you are over 30 miles away, the PA Smart Grant will cover hotel on Monday Night.
With your registration, please fill out this travel pre-approval form from Millersville university website and upload it to the registration form.

Steps for Travel Reimbursement for PA Smart Grant Program in 2023 & 2024

Open the Travel Request Form (1) from the website above.
When you open this document, to auto generates a unique number called the “Travel Request #” at the top of the form. This will be used on the Travel Expense Voucher below.

Complete the highlighted areas on both documents
(1) Travel Request Form should be completed before the June Pittsburgh Tech Tours (please go ahead and book your hotel, you don’t need to wait until approval, but if you are booking a airplane flight, please wait for approval back before booking your flight)
Note: the miles reimbursement rate is calculated automatically
Do not forget to SIGN in the indicated areas!
TIPS
The Travel request # you need for the Travel Expense Voucer is found on the top of the Travel Request Form. For this example it is 027171 (as shown in the Travel request at the top right corner)
Justification for travel and Purpose of travel: “PA Smart + (event location and name)”
Leave blank if something doesn’t apply
CHECKLIST
Travel request form (Send prior to event. Unless you are requesting a flight instead of mileage, please go ahead and book your hotel, you don’t need to wait until approval)
W9 (If you attended a PA Smart Program in 2023 or 2024 and already submitted your W9, we not not need it again).
Itemized Receipts
Travel expense voucher form (will need the number from the above Travel Request Form to complete the Voucher Form)
