Instructions for Completing the NCCAT BTOY Portfolio:

The nominee must meet the general qualifications as outlined in this nomination package.

• All information requested in the nomination form must be provided.
• Signatures of the nominee and the nominee’s principal MUST be provided.
• Responses must be limited to the number of pages requested in each section. Type size must be no smaller than 12 point. In order to provide all nominees with an equal opportunity, only the number of pages requested will be accepted. The regional selection committee will not consider any additional pages and/or materials submitted.
• Section I, must be filled out on the form provided.
• Section II and III, the essay topics to be completed by Nominee and Administrator/Colleague (i.e., Interaction with Students, etc.) must appear at the top of each page.
• Section IV, include three letters of support from among any of the following: professors, cooperating teachers, administrators, colleagues, parents or students.

The nominee's name must be received by September 13, 2019. Please submit your district's nominee here.

The portfolio must be received by October 15, 2019. Submit completed portfolios by emailing to darcy.grimes@nccat.org.
