Required Materials

Collect the following materials before you apply through our Google Form:

• Verification of employment: CL-777 FORM

You must either have an offer of employment or be currently employed in a full or part-time administrative position as defined in Title 5 of the California Code of Regulations section 80054(g)  

• Screenshot of your credential from your CTC profile page
