Athletic PE/PF Waiver

If you are an athlete on an FHSAA sports team, you can fill out this waiver AFTER YOU HAVE FINISHED 2 SEASONS, and get your coaches to sign it and then you can get the 1 credit of PE needed for graduation (0.5 personal fitness and 0.5 PE). Turn it in to your counselor once it is signed by your coaches.
