Giving and Receiving feedback is a fundamental part of Nursing Practice
Practice Supervisors are required to provide direct feedback to you on your conduct and achievement in relation to professionalism, proficiencies and skills.
This will also include where they don’t think the achievement has been met, or could be improved on through constructive feedback and the formulation of action plans.

Patient/Service User/Carer Feedback Form
Feedback is a form of evidence which can demonstrate how you meet the required proficiencies.
Within the MYEPAD there are 4 different feedback forms which can be utilised, you need to choose the most appropriate to relate to the person / child providing you with the feedback.
You will need to discuss with your practice supervisor / practice assessor when you have identified a person / child you would like to request feedback from. Your practice supervisor/practice assessor should obtain consent from patients/service users/ carers who should feel able to decline to participate.
When you have gained feedback you need to reflect upon this. This is a good time to link your proficiencies and identify your growth and development.
This will help to inform your final assessment.
