This web page is designed to help you understand the rules around school and child care immunizations. The page has all the forms families need to meet immunization requirements.
Mount Vernon School District manages student immunization records using the School and Childcare Immunization Module, an online system provided by the Washington State Department of Health.
Most children born and/or vaccinated in Washington already have their information in the system. You can access your child’s record at any time by signing up for MyIR athttps://myirmobile.com/.
Students who are missing vaccination records will be excluded from school, and will not be allowed to resume attendance until all records are received.
If your child has vaccines that are not in the system, we can enter them for you! We are asking all parents to sign their child’s Certificate Immunization Status (CIS) form, giving us consent to enter those missing vaccines. If you decline consent, your child’s immunization record will be incomplete. You may ask your doctor to enter missing vaccinations for you.

Proof of vaccinations must be medically verified. That means that it comes from an official source, like a:
doctor's office: printout, stamp, signature on the vaccination card
pharmacy: printout
government agency: military, health department, MyIR website (for vaccinations given in WA, AZ, LA, MD, or WV)
Families may also submit completed immunization exemption forms. See below for details.
Your school will send you a letter that lists the missing vaccines or records for you child.
You are welcome to call your school during regular school hours to get a list of needed vaccines.
Children entering 7th grade are required to have a booster dose of tetanus, diptheria, pertussis (Tdap). This dose must be given after your child's 10th birthday.

There are a few ways you can access your family's immunization information:
Sign up for MyIR Mobile at MyIRmobile.com to view and print your family's immunization information, including COVID vaccination.
Request a complete immunization record from your healthcare provider.
Request a complete immunization record from the Department of Health.
For vaccine records you keep yourself, that do not have a provider stamp or signature:
Take the records to your child's doctor and have the doctor write a letter stating that these vaccines are valid doses.
Washington State Law allows parents or guardians to exempt their child from the school or childcare immunization requirements. Exemptions may be claimed for personal/philosophical, religious or medical reasons. Measles, mumps, and rubella may not be exempted for personal/philosophical reasons. To request an exemption a completed Certificate of Exemption must be submitted to the school or child care center.
