A message from the school nurse:
Hello and welcome to Mount,
We are looking forward to welcoming your child to Mount in the fall. Rhode Island Department of Health requires all students to be up to date with immunizations and physical exams. Upon enrollment, a copy of your student's current (within the last 12 months) physical exam with immunization list should be sent to the school health office. If your child has a physical scheduled for the summer, we ask that the most recent physical and vaccine list is submitted upon enrollment. This allows staff to input the dozens of immunizations for each student before the health office closes for the summer. Please note No schedules for classes will be distributed to any student who has not submitted these items.
Students entering grade 7 and above must meet the following requirements:
HPV: 1 dose is required for grade 7 entry. This is mandated in the State of Rhode Island. If you elect not to get this vaccine for religious reasons, please complete the Religious Immunization Exemption Certificate.
Meningitis vaccine
2nd varicella vaccine or documentation that your child has had the chickenpox
The physical examination and the immunizations can be on the papers that the doctor gives you and does not need to be on the State of RI School Physical form specifically.
The following must also be submitted from the parent portal (when you log into the portal at www.mtstcharles.org and click on myMSC, you will see a yellow banner at the top of the page. Click on the banner to be directed to the forms):
Medication Authorization Form. This provides parent permission to have medication delivered at school. It pertains to a small group of over-the-counter medicines that may be administered during the school day.
School Health Office Form. This form lists documentation requirements pertaining to immunizations and physical exams and provides the schedule of state-mandated health screenings.
Student Health Information Form. This important form allows parents to list medical history, allergies, and other health concerns. Information is provided here regarding dismissals due to illness and emergencies.
RESIDENT STUDENTS ONLY: Resident Student Medication Authorization Form. This is specific for residential students and authorizes certain medications to be delivered in the residence hall.

The following forms may be delivered in person, mailed or faxed to the health office at (401) 200-8951.
Medication Order Form. This is to be filled out by a licensed prescriber and has a second page for written parental permission. This form is necessary for any prescription medication or over-the-counter medication which is not authorized on the portal “Medication Authorization” form.
State of Rhode Island Physical Form. All incoming students as well as all students in grades 7 and 12 must submit this form, or another form completed by his/her physician which contains the same information. Resident Students please note: this copy is to be provided to the school health office, in addition to what may be provided to the residential staff or USA Hockey.
Religious Immunization Exemption Certificate. This form, when completed, may be presented in lieu of a complete immunization record, if receiving such immunizations is contrary to your religious beliefs.
Allergy Action Plan Form. Students with life-threatening allergies who carry or have the health office keep epinephrine, are asked to provide an Allergy Action Plan. Many prescribers have their own forms, which are acceptable; if not you may submit this form.
